"These are mistakes that should never happen, regardless of the hospital", Mobley said.

Last year, the medical center received B's in both of its bi-annual grading evaluations by Leapfrog; it had A's in 2017, and B's in 2016.

Additionally, the network has more than 500 patient care locations throughout the state which include ambulatory care centers, surgery centers, home health services, long-term care and assisted living communities, ambulance services, lifesaving air medical transportation, fitness and wellness centers, rehabilitation centers, urgent care centers and physician practice locations.

An estimated 160,000 hospital patients die a year from the avoidable medical errors a year.

"The staff has done an incredible job focusing on our community and our patients", said Chuck Hays, MaineGeneral Health's president and chief executive officer.

"We focus on driving improvements in care delivery and patient satisfaction every day".

Impressively, North Suburban has the strongest record of quality of any hospital in the North Denver Metro area and has received several other quality awards and recognition recently. Northwestern Memorial earned three out of five stars, and Rush got four stars.

"We appreciate the ongoing efforts by Leapfrog to improve the usefulness and accessibility of information for consumers", Megan McCann, a Northwestern Medicine spokeswoman, said in a statement.

Earlier this year, the system, run by the federal Centers for Medicare & Medicaid Services, gave University of Chicago and Loyola University medical centers only two out of five stars.

The grade makes CMMC one of Leapfrog's highest-rated hospitals in the state.

According to an overview of how CMS rates hospitals, its "rating summarizes a variety of measures across seven areas of quality into a single star rating for each hospital".

To advance patient safety performance, KRMC uses industry-wide safety standards and measures.

UNC Medical Center in Chapel Hill, UNC REX Healthcare in Raleigh, Johnston Health in Clayton, UNC Rockingham Health in Eden, and Wayne UNC Health in Goldsboro all received top marks on Leapfrog's Spring 2019 report card.

"These scores highlight our efforts to improve overall patient safety and empower staff to spot potentially unsafe situations, including hospital-acquired infections", says Scott Kashman, Lee Health chief acute care officer, in the statement. The Hospital Safety Grade is created to give consumers information they can use to make healthcare decisions for themselves or a loved one. "The bad news is that there's still a lot of needless death and harm in American hospitals", said Leah Binder, president, and CEO of the Leapfrog Group, a nonprofit organization that assesses hospitals through public reporting initiatives.
